Start by completing our online application.
Once you online application is received you will receive an email with the ChiroHealthUSA provider agreement. Please review and sign the agreement electronically. Upon receipt of your signed agreement, your application information will be sent to the consultant we have retained at no cost to you. She will be contacting you by phone and email to set up a time to review your fee schedule and financial policy. You will establish your ChiroHealthUSA fees during this appointment. Before your appointment with the consultant, you will need to provide the following documents:
- Actual Fee Schedule (This may be printed from your software) This information will be needed for your appointment with the consultant.
- Premise Insurance Declaration page with current dates of coverage
- Malpractice Insurance Declaration page with current dates of coverage for each provider
